Page 1: double-12 set, 13 rounds (12|12 down to 0|0).
Page 2: double-15 set, 16 rounds (15|15 down to 0|0).

How to use it

Write each player's name at the top of a column.
At the end of each round, write down the dots left in each player's hand. The player who went out writes 0.
Circle 0 or 50 at the top so everyone agrees what the double blank is worth.
Add up each column after the last round. The lowest total wins.
